Bring your ID/insurance card, medication list (or bottles), recent labs/imaging, and names of your current doctors. We will review kidney function trends, blood pressure, urine testing (if needed), and create a plan with clear next steps.
CKD means reduced kidney function over time. Many patients feel well early on. Goals are to slow progression, control blood pressure and diabetes, reduce protein in urine (when present), and prevent complications like anemia and bone/mineral problems.
Most kidney patients benefit from lowering sodium (salt). Potassium, phosphorus, and fluid restrictions depend on your labs and stage of kidney disease—do not restrict these unless your care team tells you to.
We coordinate directly with your dialysis unit for monthly rounds, medication adjustments, and lab review. Contact your dialysis nurse first for urgent dialysis-related symptoms; contact our office for ongoing care questions and follow-up planning.
Some plans require a referral from your primary care physician. If prior authorization is needed for tests, our office will guide you through the process.
If labs are ordered, complete them 1–2 weeks before your follow-up (unless instructed otherwise). Results are reviewed, and significant abnormalities are addressed promptly.
Request refills during business hours and allow 2–3 business days when possible. Controlled substances are not managed by our nephrology office unless explicitly discussed.
If you were recently hospitalized, schedule follow-up within 1–2 weeks (or sooner if instructed). Seek emergency care for severe shortness of breath, chest pain, confusion, fainting etc.
